By showing calculation, suggest a recommendation to the medical team & give a reason for your recommendation. b) Increasing dose of phenytoin must be done with caution. Give a reason. ST, a 10-year-old, 30kg child, accidentally ‘drank’ one & a half bottle of paracetamol syrup (1 bottle = 100 ml, 250 mg/5 ml) at 10am, 01/09/2019. ST was admitted to hospital at 230pm on the same day. A TDM was done at 3pm and the Paracetamol level was 189 mg/L. Past medical & medication history: healthy & not on any medication. Other lab value: ALT: 46 units/L (normal 8-36); Scr:1.0 mg/dL (normal: 0.5-1.5) a) Determine the threshold for liver toxicity. b) Calculate the amount of Paracetamol ingested in gram (g). c) Is ST at risk of hepatotoxicity? Explain your answer d) Based on the nomogram, determine whether NAC infusion is indicated for ST.
